Hard copy/offline applications will not be accepted under any circumstances. Candidates must login using their PET-2025 Registration Number via OTP sent to their registered mobile/email. All personal information filled during PET-2025 (name, father's name, category, EWS, reservation details, DOB, gender, marital status, contact number, email etc.) will be pre-filled and auto-displayed - these cannot be modified. Candidates must fill in educational qualification details in Section-II of the application. After filling all sections and entering the Verification Code, clicking "Submit" will display the Application Form with an 11-digit registration number - candidates must take a printout and keep it safely. The application form correction window will be open up to 04-08-2026.

Important Instructions
- Only candidates who appeared in PET-2025 and hold a valid UPSSSC score card are eligible to apply - others need not apply. Only online applications through the official UPSSSC portal will be accepted; hard copy applications will be rejected. All reserved category candidates of UP must mention their category in the application form; only the most beneficial single relaxation will be granted if multiple categories are claimed. Candidates not domiciled in UP will not be entitled to reservation benefits and will be treated as Unreserved (General). The date of birth as recorded in the High School (Madhyamik) certificate is the only accepted proof; no other document will be accepted. EWS candidates must hold a certificate based on financial year 2025-26 income, valid for FY 2026-27, issued between 01-04-2026 and 28-07-2026; the certificate must be in the prescribed format as per UP Govt. Office Letter No. 0-3/2019/4/1/2002/ka-2/19 T.C.-II dated 14-03-2019. Divyangjan (PH) candidates must ensure their disability certificate is in the updated prescribed format clearly mentioning the disability category/sub-category and percentage, issued by the competent authority as per UP Govt. Office Letter No. 5/2022/18/1/2008/47/ka-2/2022 dated 18-04-2022. For female candidates of UP, only caste certificate issued from the father's side will be accepted. Ex-Servicemen claiming reservation must verify that they possess a certificate as per the prescribed format of the UP Government (GR dated 21 April 2015). Government/Semi-Government employees must obtain an NOC from their employer before participating in the selection process; they must produce it when demanded by the Commission. Married male candidates with more than one living wife, or female candidates who have married a man with an existing living wife, are not eligible - unless exempted by the Governor. If a candidate submits more than one application, only the application with the latest Application Number will be considered; fee paid against other applications will not be adjusted. Mobile phones and electronic gadgets are strictly prohibited inside examination premises. Negative marking of 1/4 mark applies for each wrong answer in the Written Examination.
